Make the narrowest complete change that satisfies the request. Read the surrounding code/docs, preserve local ownership, and do not clean unrelated dirt.

Positive Version

Surgical does not mean timid. It means scoped: delete drift, fix the actual invariant, update the right generated surfaces, and stop when the bounded job is complete.

Failure Mode

The rule fails when the agent modifies nearby files because they looked tempting, or when it hides a behavior change inside cosmetic churn.
